ABLE Bodies Balance Training

ABLE Bodies Balance Training offers an activity-based program to improve balance and mobility for both fit and frail older adults. This practical instructor's guide provides more than 130 balance and mobility exercises that consider flexibility, strength, and cardiorespiratory endurance. The exercises enhance older adults' abilities to maintain balance in completing their everyday tasks, thereby fostering increased self-confidence, reducing the occurrence of falls, and improving quality of life.

The text is based on ABLE Bodies techniques, which were proven effective in a randomized, controlled study funded by the National Blueprint and Active Aging Partnership. Results showed that ABLE Bodies training significantly improved balance, mobility, activity levels, gait speed, flexibility, and strength for participants 70 years of age and older living in retirement and assisted living facilities.

ABLE Bodies Balance Training uses current research and a component-based approach to balance training. Instructors are encouraged to use activities covering all five components of the program: flexibility, posture and core stability, strength, cardiorespiratory endurance, and balance and mobility. The exercises and activities are easily implemented with the use of existing facilities and inexpensive equipment. They also encourage fun and social interaction, helping instructors to create and maintain an energized and positive environment that improves communication, motivation, and overall progress. The program may be used in group or individual settings and can be customized according to level of experience:

  • Beginning instructors can follow the 16-week sessionplan as a well-balanced training program that safely progress older adults through the exercises. The material is complete and may extend to a yearlong program.
  • More experienced instructors may select exercises and activities from each of the five component categories to meet the individual needs of their older adult clients. The component-based organization of the text allows instructors to easily incorporate both exercises and conceptual ideas in fun, engaging, and creative ways into their existing programs.
As a bonus, access to a dedicted ABLE Bodies Balance Training Web site is included with the book. It offers 15 downloadable activity handouts that instructors can print out and distribute to patients or clients for use at home. It also offers downloadable handouts of all the balance training activities in the book-over 130 conceptual ideas and activities to choose from either for planning their own sessions or for aiding their delivery of the 16-week session plan. The text also offers a variety of tools, including step-by-step instructions, sample phrasing to use when encouraging participants, ideas on making activities progressively more difficult, and tips on promoting safety while performing the exercises.

The proven exercises and conceptual activities found in ABLE Bodies Balance Training offer both fit and frail seniors increased independence in daily living. By incorporating the ABLE Bodies Balance Training program into their work, fitness and health care professionals will educate and motivate older adults to increase stability and improve self-confidence and health.

This book is designed to demystify cancer medicine and provide solid advice to physicians who care for cancer patients.

This book is an overview of the essentials of clinical oncology with the case presentations of some of the more complex issues and decision-making in oncology, The purpose is to provide the essentials of oncologic diagnosis and treatment through didactic material and case presentations. These are appropriate objectives, particularly in view of our aging population and with malignancy as the second highest cause of mortality in the U.S. The author's objectives are successfully met. The audience, according to the author, includes medical students, residents, and family practice and internal medicine practitioners. The audience is appropriate. The author is a credible authority in the field of hematology/oncology. In the first half of the text, general information regarding the pathogenesis, diagnosis, and staging of malignancy is presented, with specific topics related to the care of the patient with malignancy. In the second half there is a discussion of the presentation, evaluation, and treatment of specific malignancies. The use of case presentations and flow diagrams (evaluation of breast lump, lung nodule) are particularly instructive and demonstrate major concepts end salient points important to the management of oncology patients. Shortcomings include a somewhat liberal list of indications for PET scanning in Table 3-12 (not currently recommended for staging in NCCN guidelines for lung cancer); Table 5-4 has reversed dosages for granisetron and ondansetron; Table 12-3 lists CMF or MF as equivalent therapy for LN- ER- breast cancer, which is no longer accurate (updated data from 1997 NSABP meeting reveal statistically significant benefits for DFS and OS for CMF over MF). Inaddition, staging tables in Chapter 3 are repeated in subsequent chapters of specific malignancies and do add to the chapter. Although presenting a comprehensive guide to the approach of the patient with malignancy for the non-specialist is a daunting task for one author, Dr. Bitran is successful. The book contains useful information regarding the presentation, evaluation, and treatment of common malignancies. Flow diagrams and case presentations are instructive for any level practitioner. Although not as complex or in-depth as edited texts with multiple authors such as Weiss' Clinical Oncology (Appleton & Lange, 1993), students, residents, and primary care practitioners will find this a handy addition to their libraries.

As suggested in the preface, although most of us would prefer to die at home, a large majority of us will die in a healthcare facility hooked up to a myriad of machines and devices. This book is a timely and needed response to society's current immersion in the milieu of the technological imperative.
Audience:This is appropriate for a broad audience and is written in a manner that will be accessible to many, including those unfamiliar with medical terminology or ethics. The book also provides enough relevant information on the topics of empathy and empirical research to be useful to healthcare professionals whose patients are facing these intricate decisions.
Features:Throughout the book, the author recounts cases from his own practice as well as famous cases to illuminate his points. He provides a useful chapter on completing advance directives and another on how to accurately assess facts and statistics on treatment outcomes. He also incorporates literary references from Shakespeare, Chekhov, and Tolstoy into the text, and provides an appendix with poems by William Carlos Williams and Anne Sexton. The author concludes with achapter addressing additional recent concerns in healthcare, including the need to provide universal healthcare and the inexorableness of rationing healthcare.
Assessment:This book is engaging, due in large part to the case studies and literary references. The concise arrangement of short chapters with numerous references makes the book manageable, but substantial enough to be valuable. The honest and realistic information is a welcome alternative to the inaccurate depictions of the end of life provided by the media.

As medical advances and lifestyle changes carry more and more Americans beyond the age of 80, decisions about quality of life and manner of death grow increasingly unavoidable. Schneiderman (Univ. of California at San Diego Medical Sch.) and geriatrician McCullough (The Little Black Book of Geriatrics ) remind us that the issues surrounding decline and death are not isolated, spectacular events happening to others, as in the Terri Schiavo case, but a part of all our lives. Here, they each discuss the need for advanced planning and thoughtful communication and care.

Schneiderman emphasizes the need for clear and current advance directives to balance the tendency of modern medicine, abetted by hopeful loved ones, to continue treatments to the detriment of the patient's comfort. Using many literary references (this M.D. is also a novelist, a short story writer, and a playwright) and calling on philosophy as well as science, Schneiderman argues that physicians need to work from an "ethic of care" that gives equal status to the relief of suffering and the restoration of health.

McCullough's concept of "slow medicine" is an example of that ethic of care in action. Having worked through these issues himself with his own mother's death, he here guides families as they move with an aging relative along what he calls the "eight stations of late life" preceding death. McCullough believes communication at this time is essential, and he emphasizes gentle, low-impact, and thoughtful care from the first signs of the body or mind's decline. Often, he points out, elderly patients can benefit more from attentive listening, conversation, touch, and a reduction in medications than fromacute interventions.
